Jason

Jason Collins

Nurse Practitioner

MSN, APRN, FNP-BC

"I was raised in Patterson, LA. Being raised by a mother that was a nurse, I knew from an early age that I wanted to have a career in healthcare. In 2001, I graduated from University of Louisiana at Lafayette with a Bachelor of Science in Nursing degree. I began working in emergency departments within Acadiana for the next year. I moved to California in 2002 to begin travel nursing. Working in many different emergency departments and trauma centers definitely prepared me for my future career as a nurse practitioner. I finished my last assignment at “Big Charity” in New Orleans working nights on the trauma side. I decided then it was time to continue my education and I graduated from the University of South Alabama in 2006.

I have been a family nurse practitioner for 15 years. In those years I have practiced in primary care, emergency medicine, otolaryngology, and urgent care. Having the opportunity to work for Sync Health has given me a new opportunity to focus on both wellness and prevention for our clients. I look forward to continuing to give my best to the Acadiana community and beyond for years to come.

I have been married to my wife Beth for almost 20 years. We have 3 children, Tyler (16), Hannah (14), and Layla (7). In my spare time (when I have some) I enjoy spending time with family, fishing, hunting, and going to sporting events."

"I was raised and currently reside in Kaplan, LA. My interest in nutrition began in my high school years when I knew I wanted to be involved in the healthcare industry to make a true impact in people's lives. After graduating from Kaplan High School in 2014, I attended the University of Louisiana at Lafayette and obtained a Bachelor of Science in Dietetics in 2018. During my studies, I quickly realized that the science of nutrition is foreign to most due to the misconstrued media and diet culture. I knew that by becoming a Registered Dietitian I could change lives, mindsets, and attitudes with nutrition knowledge.

I furthered my education by completing my Master of Science in Nutritional Sciences degree as well as my Dietetic Internship at Franciscan Missionaries of Our Lady University in Baton Rouge, LA in 2019.

I joined the Sync Health team as their Registered Dietitian in 2020 where I provide our members with nutrition guidance, knowledge and education to choose foods that will fuel their bodies and are sustainable long term. I recognize that nutrition plays such a large role in the overall health and medical management of our members. I enjoy exploring each member’s nutrition needs so they can reach optimal health through our personalized approach. I am currently studying to become a Certified Leap Therapist (CLT) to help our members better understand how their food sensitivities we have identified through testing we offer are affecting their health.

In my spare time, I enjoy traveling, hiking, and spending time with friends and family. I am married to Slade Boullion and we are expecting our first child in November 2021."

"I have worked in many fields over the last 25 years which includes : Financial Companies, Banking, Substitute Teacher, ELA interventionist, Optician, and was co-owner and operator of my own business.

I was born in Abbeville, La. A year after graduating from Patterson High School I married and moved to New Iberia, La. Five years after getting married I had my first daughter and became a stay at home mom. Four years following, my second daughter was born. Most of my time was dedicated to my daughters and their education.

I joined the SYNC Health Team in February 2020. I’m a very outgoing person who loves to meet and talk to people. As I get older I realize how important it is to keep up with my preventive health appointments. Working here I get a chance to help others in that aspect of their lives as well.

In my spare time I enjoy spending it with my daughters, family and friends, reading, and spoiling my dog Sasha and cat Luna Belle."

"I am a native of Acadiana, with 25+ years of experience as a Registered Nurse. Initially, I graduated from University of Southwest Louisiana in 1990 with a Bachelor of Arts in Secondary Education with a major in English Education and a minor in Library Science.
Realizing that my passion was in healthcare, I attended University of Southwestern Louisiana again in 1992 and graduated with a Bachelor of Science in Nursing. My love of fitness and health has become a hobby for me and as a lifelong student I continue to learn all I can about health, fitness and wellness.

I started working at Sync Health in November 2019 and became a Certified Health Coach through the Dr. Sears Wellness Institute. Recently I became certified as a Personal Fitness Trainer at NESTA/Spencer Institute and also obtained certifications in Stress management and Sleep Science with Spencer Institute as well. At Sync Health, I strive to get our members enthusiastic about their health and assist them in making their health and well-being a priority. I hope my passion and influence can change the lives of others.

In my spare time I love exercising- especially running half marathons. I also enjoy gardening, traveling, reading and spending quality time with my husband of 15 yrs, Glenn. Much of my time is also spent with our rescue dog- Max and our 3 cats Olive, Reesi and Pete."

"I grew up in Ville Platte, LA and have been living in the Lafayette/Youngsville area for the past 28 years. I have been a Respiratory Therapist since 1993 and I joined the Sync Health Care Team as one of their Certified Health Coaches in 2019. I’ve since then received my Certification as a Personal Trainer, certification in Sleep Science and in Stress Management all through NESTA/Spencer Institute.

It’s an honor and privilege to do what I do! At Sync Health, I help guide and educate our members regarding their medical health and with knowledge of how nutrition, physical activity, sleep and stress play a huge role in prevention.

A few of my “favorite” things are - my grandbabies, relaxing and spending time on the lake with my loved ones, running, exercising, teaching boot camp, and selling real estate. I am also a local artist and particularly love painting Louisiana art."
